By participating in Chosen Family Kickball Night, all participants (adults and children) acknowledge and accept the risks associated with physical activity. By submitting this form, you agree to release Chosen Sports Leagues Inc., Liberty Union High School District, La Paloma High School, its staff, volunteers, and affiliated partners from any and all liability for injuries, accidents, or damages that may occur before, during, or after the event.
Participation is voluntary, and families are encouraged to play safely, stay hydrated, and look out for one another.
